Who is eligible for a wholesale account?

Medical Practices and Independent Pharmacies.

How do I sign up for an account?

Signing up for an account is simple. Create an account and select "Wholesale". You will need to submit a W-9 and a tax exemption or resell certificate to be considered for the wholesale program. After you fill out the form, our team will verify your information and grant you wholesale portal access. From there you will then see prices, order, access product studies and more. Please Note: If you are located in:
Alaska, Colorado, Delaware, Oklahoma, or Wyoming your state does not require a
resale certificate. Please upload your W-9 twice.

Why do you need my W-9 and either a tax exemption form or resell certificate?

To make sure you are who you say you are, and that you are
legally able to resell product in your practice, we need to collect a W-9 and either a tax exemption form or a resell certificate.

How do I order product?

You can order product through your wholesale account on our
website at: https://eosera.com/pages/wholesale-registration-form

Is there a minimum or maximum order quantity?

The minimum order quantity is 12 of each unit, EXCEPT for
Wax Blaster MD Pro (minimum order of 8 units), Ear View MD (minimum order of 6 units), and Ear Clear MD (minimum order of 6 units).
You must order each product (except for Wax Blaster MD Pro, Ear View MD, and Ear Clear MD) in multiples of 12. If you order a case (24 units) you will receive a discount.

Note:
The Wax Blaster MD Pro case pack is 16 units, the Ear View MD case pack is 12
units, and the Ear Clear MD case pack is 12 units.

There is no maximum order quantity.

How much should I sell products for?

It is required that your practice sells eosera products for their suggested retail price. The listed MSRP of the product is the minimum amount you can charge for the products when reselling, though you are able to charge more if desired. Going below the specified amount is considered a breach in the agreed upon terms and conditions.

Can I pay with credit terms instead of a credit card?

If you’d like to pay with credit terms, you may contact us
by emailing hello@eosera.com to apply.

How long is normal shipping time?

Shipping time depends on many factors, such as where you are
shipping to and the preferred shipping method you select during checkout. Shipping can take up to 5-7 days and be as quick as 1 day.

Can I expedite my shipping?

Yes, you may expedite your shipping by selecting your preferred shipping method during checkout.

Can you ship internationally?

No, we cannot ship anywhere outside of the United States of America.

How do I cancel my order?

Cancellations are acceptable if the order has not shipped yet. If the order has shipped, we are unable to cancel the order and it may be returned unopened and unused within 14 days of receipt. The purchaser will be responsible for freight on returned products and will incur a 15% restocking fee. The purchaser must obtain a Returned Goods Authorization form from eosera via phone 844-732-7929 or email hello@eosera.com

The Returned Goods Authorization form must include: 

- Product name 

- Lot number 

- Quantity of product 

- Wholesale account number. Authorization expires 15 days from Issue.

Can I return opened product?

No, you cannot return opened product or product received
within 14 days of purchase.

What is your return Policy?

Returned Goods

· Products
are returnable within 14 days of receipt. After 14 days, products are not returnable.

· Product must be in resalable
condition (unopened and undamaged), and the purchaser will be responsible for freight on the return product(s) plus a 15% inspection/restocking fee. 

·Refunds will be processed within 10 days after the
product return is received in the same manner of payment as the original sale transaction, less the 15% inspection/restocking fee.

· For all returns, the purchaser
must obtain a Returned Goods Authorization form from Eosera via phone 844-732-7929 or email orders@eosera.com

 

The Returned Goods Authorization form must include: 

- Product name 

- Lot number 

- Quantity of product 

- Wholesale account number. Authorization expires 15 days from Issue.
